Said order form is not present in this lot, but book is otherwise intact and in the original tube. Measurements are approximated as the book has been stored in the tube so long we had trouble getting it to lie flat. Back cover has several deep scratches, as if someone had used it as a cutting surface. Tube shows moderate shelf wear
